20-30 minutes long interview with patient and/or with their care givers
During mediation reviews, our goal is to make patient understand their medications, why and what they are taking.
Identify and potential problems and interactions
Create an action plan for medication management and follow-up periodically to ensure plan is being followed.
Collaborate with other healthcare team to help achieve patient’s goal.
Pharmacist will provide best possible medication history at the end of interview.
